Continuity Care Home Nurses is a full service licensed home health agency which provides nursing and other therapeutic services to patients of all ages in the comfort of their own homes. Home health care needs ranging from highly technical nursing to personal assistance are met by our professional, highly qualified, experienced and carefully screened staff. 

Licensed nursing services are provided according to an individualized treatment plan under the direction of the patient’s physician and supervised by a registered nurse. We will also provide support for other health professionals in the home through our multi-disciplinary approach- including physical therapists, occupational therapists, speech therapy, dietary consultation, medical social workers, and certified home health aides as ordered by the patient’s doctor.
